SERVICES

Harmony Homes Adult Residential Facility will provide a structured system of care that will promote stability and independent living.

Services include but are not limited to:

·       24-hour Residential Care & Supervision

·       Medical & Dental Services, Medication Support

·       Facilitation of Adaptive Equipment, Medical Equipment & Supplies

·       Psychological and Psychiatric Services

·       Positive Behavior Support Intervention with Replacement Behavior and Equivalency Training

·       Coordination of Physical & Occupational Therapy appointments

·       Educational Support

·       Social Skills Training, Recreational activities (games, sports and exercise)

·       Personal Attendant/Assistance with ADL tasks (e.g., bathing, grooming, toileting, cooking, etc.)

·        Laundry Services, Clean Linen (Staff Support for Bed Making)

·       Nutritional Services (three prepared meals per day, including snacks in between)

In addition to providing the services stated above, each resident will be given the choice of attending an outside day program (e.g., Behavioral Management Day Center, Work Program, etc.). The selected program will be based on the Client’s interest, level of functioning and maladaptive behaviors. Of course, it is our policy that 24-hour coverage be provided. Therefore, day program attendance will not be mandatory, and staff will be available in the event of emergencies.
